To complete a request for a Taxpayer, the company must provide the following information with a copy of the lien in question:

• Full Name
• Social Security Number
• Date of Birth

Please submit your request on the My Nevada Tax website. Under the “Additional Services” tab please select “Request a Lien Demand” and complete the form.
